How to Create an Account & Access Patient Portal

Step 1:

  • Go to
  • Click on Patient Portal

Step 2:

  • Create an account by using the Register Button

Step 3:

  • Enter your first & last names, and email address
  • Click Register
  • Go to the email inbox you used to create the account

Step 4:

Once you have located the email from LoginClinics Patient Portal

  • Click to “Create your account”

Step 5:

From this screen you will need to enter the following

  • Password ( Instructions next page)
  • Date of Birth (Format – 01/01/2000)
  • Put check in the box to agree w/ Terms and Conditions
  • Then click register

Password Requirements:

Password requirements are as follows:

  1. Must be at least 8 characters in length
  2. Must use at least 1 Upper case & 1 Lower Case letter
  3. Must use 1 number
  4. Must use 1 Symbol, (e.g. @, $, !, &, #)
  5. Less than 4 repeated Characters ( e.g. AAA)

You will see at the top if you have met all of the password requirements.

  • All requirements will have a Green check
  • Then Click the “REGISTER” Button

Step 6:

For security reasons, you will be prompted to enter a verification code

  • Click send code ( This will be sent to the email address you used during registration)

Step 7:

  • Open Validation Code Email ( This may come through Junk/Spam inbox)
  • Codes will expire in 15 minutes or less

Step 8:

  • Enter the Validation code received into the space provided
  • Click Verify

Step 9:

  • Once all Two-Factor Authentication steps are completed
  • Click Continue

Step 10:

  • You have successfully created your LoginClinics Patient Portal account, where you can request/schedule appointments, send messages, complete forms, pay statements and more.
