LoginClinics is a woman-owned and operated medical practice located in Wake Forest, NC. Managing member, Jaclyn Qualter, RN, MS, APRN-BC, A-CTTP is a nurse practitioner and certified telehealth and telemedicine provider with over twenty years healthcare experience. LoginClinics is pending acceptance as an EDWOSB (economically disadvantaged women owned small business) in both North Carolina and Virginia.
Our firefighter physicals can be performed on site at the firehouse, or in our office building. We can do this in 1 or 2 days’ time, all together, or staggered. After the testing is completed, LoginClinics will provide individual clearance letters to the firefighter and the department. Tests include the following:

A DOT physical is a checkup that’s required for commercial vehicle drivers by the Federal Motor Carrier Safety Administration (FMCSA). This exam makes sure you’re able to meet the physical requirements of your job. This includes being able to tolerate the health risks posed by sitting for long periods of time. Our CDL medical examiner will conduct the test and provide documentation of its results to you.
We offer 5, 9 and 12 rapid urine drug screenings. These rapid tests will identify the presence or absence of different classes of commonly abused or misused drugs or their metabolites. This drug screen provides results within 10-15 minutes and is most commonly used for Pre-Employment applicants. On average, most drugs are detectable in urine for approximately five days after the most recent use or exposure. A “non-negative” result for a drug or drug metabolite is considered evidence of use or exposure in that time period. If further confirmation is needed, the urine specimen can be sent to Quest Labs for further testing. This test requires a standard urine sample that can be collected at LoginClinics or at the place of business.
DOT drug and alcohol testing requirements are set by the U.S. Department of Transportation. DOT specifies the requirements for compliance with this set of regulations for employers of safety-sensitive employees in transportation industries including trucking, shipping, aviation, rail, transit, and merchant marines. Companies in these transportation industries are required to give their employees a DOT drug test before hire, to participate in random pools or a drug testing consortium. DOT conducts random and scheduled audits to ensure that companies adhere to these testing guidelines and maintain proper records. Our local office, on-site testing capabilities, and online portal, which retains all of your testing records, makes it easy to stay in compliance.

For many small business owners, offering an affordable medical insurance plan is not possible. The plan price is typically too expensive for the employer and/or the employee. LoginClinics believes small business owners can attract and retain more qualified employees if they are able to offer some type of health insurance benefit. Because of this, LoginClinics offers an employer-sponsored telemedicine subscription for employees and their immediate family members. For just $8 per employee, per month, employees can utilize LoginClinics’ telemedicine services, which include evaluation, diagnosis and treatment for minor illnesses needs. (Sore throat, cough and congestion, rashes, fever, nausea and vomiting, sinus infections, urinary tract infections, cold and flu, bronchitis, allergies, kidney stone pain, shingles, medical opinion/evaluation, temporary medication refills, backache, laryngitis, pink eye, yeast infections, vaginal issues, etc.)
For $14/month, their entire family can be covered. It is up to the employer to decide which plan they would like to sponsor for their employees. LoginClinics bills the employer monthly, for all those enrolled. Enrollment can change month-to-month as needed, or a flat rate can be provided monthly and re-evaluated yearly. The employee is responsible for a $20 co-pay at the time of service.
Additionally, if your employees are a part of our telemedicine subscription plan, those 13 years of age and over can utilize our clinic in-person and receive a discounted rate of 10% off our self-pay fee schedule just by being your employee!

CPR certification is provided through the American Heart Association. The AHA develops science-based CPR guidelines and is the leader in first aid, CPR, and AED training.
The Heartsaver course is for individuals that are non-medical. This course reviews and implements the proper steps on how to perform adult, child, and infant CPR. LoginClinics will also include basic first aid and AED training for no extra charge if requested. The length of the course typically takes 4-5 hours.
The BLS course is the foundational course for healthcare professionals and first responders who provide care to patients in a variety of in-facility and prehospital settings. This course will only take 2 hours to complete.

Both prices include your AHA certification card and rescue mask. Each certification is valid for 2 years.
Our unique business model and core telemedicine practice allows us to rapidly adapt to unique and changing client requirements. LoginClinics will bring this flexibility to any EHS project. We will provide dedicated highly qualified staff on-site at each assigned location to provide the necessary and agreed upon healthcare services. In addition to our dedicated onsite staff, LoginClinics will provide rapid remote support via Telemedicine to further triage the illness/injury to ensure the best next-step treatment options for the employee and minimize lost time and additional expenses.
Providing remote telemedicine support dramatically reduces the need for off-site physician visits and can increase overall employee health and morale. The average ER or walk-in clinic visit results in unpredictable costs and several hours/days of lost time. Our partners will realize significant benefits over the course of the project with our unique combination of rapid on-site incident response combined with remote telemedicine support. If it is determined that an employee must be seen by an off-site provider, LoginClinics will endeavor to manage the process to the benefit of both the employee and our partner.

Jaclyn is the founder and managing member of LoginClinics. Ms. Qualter has twenty years of combined experience in emergency medicine, cardiology, vascular surgery and trauma medicine. She is also a certified telehealth and telemedicine provider. She is passionate about health, individuals, quality, employee satisfaction and client success.
Tyler is an active paramedic and field training officer for Franklin County, EMS. He has direct managerial experience and is currently studying healthcare administration at Liberty University.
LoginClinics staff will work with our partner’s project management to establish a regular communication cadence. We expect communication protocols to evolve based on project stage and circumstances on the ground.

At the end of each quarter, activity summary reports will be provided to our partner. These reports will include tracking of incidents, including type of injury and treatment, number of referrals, telemedicine visits, off-site care and telemedicine use.
Our staff’s primary goal is to support EHS staff on site in their efforts to promote workplace safety by a variety of means and methods. We provide non-contact temperature checks at the start of each workday. We also perform daily walks around the construction site to ensure employee compliance with prescribed safety equipment and clothing. Our staff identifies and addresses any workplace violations or compliance issues and works with the employee(s) and EHS staff to rectify the issue(s).
If an employee is injured on the job, we will document the incident in our EHR, and triage appropriately according to our partner’s policy (onsite care, telemedicine, urgent care, ER). Our primary goal is to provide appropriate medical treatment but also to minimize the time lost in the workplace. To support our main goal, we use telemedicine (medical care from a distance) when necessary to keep the employee on the job site. Next, we maintain contact with said employee(s) to ensure they are treated appropriately and not in excess, as well as making sure they are capable in the fullest capacity to safely return to work as quickly as possible; at all times ensuring compliance with local and corporate occupational safety and environmental regulations and procedures.
All of our employees working on site will be required to obtain training in both OSHA 30 and OSHA Injury and Illness Recordkeeping and Reporting Requirements.
We offer onsite drug and alcohol screening in compliance with local government standards. Urine drug tests, saliva blood tests and alcohol breath tests will be utilized post incident.
LoginClinics maintains a medical supply room and will stock necessary supplies commensurate with requirements of the project. Supplies and capabilities including, but not limited to:
LoginClinics offers onsite yearly wellness screenings for our partners employees and contractors if desired. These screenings will include measurement of height, weight, body mass index, blood pressure and general screening labs such as glucose, lipid panel and PSA. The results will be mailed to the patient after they are processed and reviewed, along with recommendations. These screenings are done on site by LoginClinics staff.
LoginClinics offers Telemedicine for individuals and families. Our partners and/or their subcontractor partners can extend affordable telemedicine healthcare plans to individuals and families. These services can be added to the contract or simply offered to employees for self-pay at corporate/group rates. This highly valuable service increases overall employee health, increases morale and reduces worker turnover.
