Program Overview

LoginClinics Concierge Medicine Program is a monthly medical membership for individuals and families who want or need quick, easily-accessible medical care and advice.

Each LoginClinics provider accepts 20-30 clients into their concierge program. This is to keep our focus on the medical care and attention that you need. We strive to provide comprehensive, caring, thorough and cost-effective medical care. LoginClinics also incorporates telemedicine services into our packages, primarily to help maintain continuity of care and continuous contact with our clients.

Goals

1. To provide convenient and thorough medical care with minimal wait time
2. To act as your healthcare liaison and coach
3. To be easily accessible via text, phone or portal (secure email)
4. To make medically necessary referrals and
5. To provide medical care that is individualized, personal and comprehensive

Plans & Pricing

There are four plans with varying access to choose from:

Urgent Care Family Plan: $50 initial enrollment, $25/month, $10 fee per use, cancel anytime.

• For families only – does not provide Primary Care.
• For all immediate family members living in the same house.
• Conditions treated: Sore throat, cough and congestion, rashes, fever, nausea and vomiting, sinus infections, urinary tract infections, cold and flu, bronchitis, allergies, kidney stone pain, shingles, medical opinion/evaluation, temporary medication refills, backache, laryngitis, pink eye, yeast infections, vaginal issues, etc.

Subscription Concierge Plan: $150 initial enrollment, $50/month, $25 yearly administration fee. 1 year contract.

• For up to 3 health conditions.
• Monthly “check-in” appointment and goal discussion/revisions.
• Up to 2 TeleHealth appointments per month.
• Additional appointments in excess of contract, $50 per incident.
• House calls $125 and required for primary care, at least yearly.

Executive Concierge Plan: $150 initial enrollment, $100/month, $25 yearly administrative fee. 1 year contract.

• Monthly “check-in” appointment and goal discussion/revision.
• Up to 4 TeleHealth appointments per month.
• Additional appointments in excess of contract, $50 per incident.
• House calls $125 and required for primary care, at least yearly.

Direct Concierge Plan: $150 initial enrollment, $150/month, $25 yearly administrative fee. 1 year contract.

• Monthly “check-in” appointment and goal discussion/revision.
• Unlimited telehealth appointments.
• HIPAA compliant text messaging privileges.
• Email available.
• House calls $125 and required for primary care, at least yearly.

What is concierge medicine?

Concierge medicine is a practice that charges patients a flat fee for personalized care and access to their health care provider. Jaclyn and Justin understand that health problems can come up at any time. By paying a subscription fee, patients have direct access to Jaclyn and Justin through determined methods of the plan that they choose. Using this service, patients don’t have to wait to schedule an appointment but can use wither email, text or phone call to get their healthcare needs addressed quickly and thoroughly.

What is the difference between concierge medicine and traditional office care?

A traditional office practice manages a high volume of patients at a time and charges for every interaction. This often comes at the cost of personalized attention.

LoginClinics in Wake Forest, North Carolina, uses the concierge model to provide convenient medical care whenever it’s needed. Concierge health care allows Jaclyn and Justin to spend more time with each patient, focus on preventive care, and offer shorter wait times in addressing symptoms.

As a patient, you can reach Jaclyn or Justin at any time, for any reason, and you don’t even have to leave your home. These plans do not take the place of your in-person PCP. These plans are in addition to them to provide you with the best medical care possible.

What can I expect from concierge medical service?

Patients subscribing to LoginClinics’ concierge medicine program don’t need an appointment and are always able to get in touch either with Jaclyn or Justin to receive quick access to their services. This is particularly useful if you have ongoing health issues.

You can expect a shorter wait time and your appointment will last as long as needed.

What are the benefits of concierge medicine?

You get care at the time you need it. No more waiting weeks to be seen. You also have an ongoing and fluid relationship with your provider so if you have a question or concern you can simply email, text or call and get that question answered. Additionally, the concierge programs focus on more of a holistic care-coordination approach, which means Jaclyn and Justin review all of your records and are able to know you and your medical history in-depth. They are your personal healthcare mentors who look at the entire picture, not just one facet. This manner of healthcare will lead to better health, less side effects and better outcomes for you.

When scheduling, choose the “enrollment” appointment type.
